Registration Process

 

You must register in the new Teva Request Management System (RMS) launched in November 2017 before submitting any requests.  For a copy of the registration guide for the new Teva RMS, please click below:

 

 

 

 

 

1. Not completing all required fields (the "Attention" field, located under "Contact First Name, last name, and title are the most commonly missed required fields)

2. Not uploading the required W-9 form

3. W-9 File size too large (Max Size 15 Megabytes)

4. Not following the user name and password guidelines

5. Email address already in use in the RMS Portal

6. In-activity during the registration process for 45 minutes or longer
